For example, amazon ads from .com amazon site should be displayed on USA/canada and a big part of the world, while amazon.co.uk/de/it/etc site should be displayed in european countries.
‘per-ad’ approach seems to be fine.

On the sidebar, we’ve put four ad rotator widgets and named each one to coincide with each of the four regions. If an advertiser in one region would also like their ad to show in another region, we can sell them that additional spot for an additional cost. A nice up sell. This gives the site visitor an easy way to find businesses in their area, to support.
